1. How many days per week do you participate in aerobic activities, such as running, swimming, cycling, etc.

2. Do you experience any of these symptoms when you exercise? Check all that apply.

  • Shortness of Breath
  • Wheezing or Noisy Breathing
  • Chest Tightness
  • Coughing
  • Trouble Getting a Deep Breath
  • Decreased Exercise Endurance

3. Do you experience these symptoms?

  • Within the first 15 minutes
  • Between 15-30 minutes after starting exercise
  • More than 30 minutes into your workout
  • Minutes after your workout

4. Have you ever stopped or avoided exercise because you had difficulty breathing?

  • Yes
  • No

5. When was the last time you had to stop or avoid exercising because you had difficulty breathing?

  • Within the past week
  • Within the past month
  • Within the past six months
  • Within the past year
